Once you have joined the Society, please take the time to fill out the information below and select submit.  Once the information is submitted, a Cremation Society of Chittenden County representative will review and file it and will be in touch with you to further discuss the details of your arrangements if necessary as well as send you a card with your membership number and the information you submitted for your records.
